medical settings with the Patient Health Questionnaire (PHQ): a diagnostic meta-analysis. 25 Smith EV. Detecting and evaluation the impact of multidimensionality using item fit statistics and principal component analysis of residuals. Unlike many fields in medicine, most clinical outcomes in psychiatry are not directly observable and cannot be captured with diagnostic tests such as blood work or imaging. In recent years, the importance of the routine use of clinical outcome assessments (patient-reported outcomes, clinician-reported outcomes, observer-reported outcomes and performance outcomes) for measuring the symptoms of disease and treatment outcomes has been increasingly emphasised. 1 Clinical outcome assessments such as the Patient Health Questionnaire-9 (PHQ-9) 2 are now commonly used in clinical research and practice to provide an assessment of a patient's severity of mood and improvement in response to treatment. 3 More broadly, as the demand increases for a broad range of mental health services to be patient-centred, clinical outcome assessments are used to capture outcomes such as sustained symptom reduction, return to full functioning and optimal patient well-being. 4 To optimise mental healthcare, clinical outcome assessments used in psychiatry should be shown to be fit for purpose. They should appropriately capture the concept of interest (e.g. depression) in the context of use (e.g. B 2016 The Authors. Summary This commentary argues the importance of robust, meaningful assessment of clinical and functional outcomes in psychiatry. Outcome assessments should be fit for the purpose of measuring relevant concepts of interest in specific clinical settings. As well, the measurement model selected to develop and test assessments can be critical for guiding care. Three types of measurement models are presented: classical test theory, item response theory, and Rasch measurement theory. To optimise current diagnostic and treatment practices in psychiatry, careful consideration of these models is warranted..

متن کاملThe Impact of Raters’ and Test Takers’ Gender on Oral Proficiency Assessment: A Case of Multifaceted Rasch Analysis
The application of Multifaceted Rasch Measurement (MFRM) in rating test takers’ oral language proficiency has been investigated in some previous studies (e.g., Winke, Gass, & Myford, 2012). However, little research so far has ever documented the effect of test takers’ genders on their oral performances and few studies have investigated the relationship between the impact of raters’ gender on th...
متن کاملApplication of the Rasch model in measuring personality disorders.
OBJECTIVE To describe item and person parameters obtained with the Rasch model, one of the item response theory models, in the assessment of personality disorders based on Millon's theory. METHOD A total of 350 people participated in the study. Age ranged from 18 to 67 years (mean ± standard deviation = 27.02±10.13), and 71.7% of the participants (n = 251) were female. Of the 350 individuals,...
متن کاملRater Errors among Peer-Assessors: Applying the Many-Facet Rasch Measurement Model
In this study, the researcher used the many-facet Rasch measurement model (MFRM) to detect two pervasive rater errors among peer-assessors rating EFL essays. The researcher also compared the ratings of peer-assessors to those of teacher assessors to gain a clearer understanding of the ratings of peer-assessors. To that end, the researcher used a fully crossed design in which all peer-assessors ...

BACKGROUND The Psychiatric Out-Patient Experiences Questionnaire (POPEQ) is an 11-item core measure of psychiatric out-patients experiences of the perceived outcome of the treatment, the quality of interaction with the clinician, and the quality of information provision. The POPEQ was found to have evidence for reliability and validity following the application of classical test theory but has ...
